1. Enter LEVY as Login Password
2. Select "Yes" when asked is Levy Restaurants correct.
3. Fill out all information until you get to employment information, then click "Begin" at bottom of page.
4. After reading each section, please take the complete test.
5. Once all questions are answered correct, click "I Agree" on the next page.
